6. Make a Formal FOI Request

- A formal FOI request should be made if you can't access the information you need through your own research or by contacting the appropriate government department or agency.
- To make a formal FOI request you will have to:
- identify the relevant government department or agency that holds the information you require;
- put your request in writing. A lawyer may be able to advise you if there is a specific form that must be filled in. In some states an application can also be lodged online;
- mention that it is an application under your state or territory FOI Act;
- specify the information you want access to and all the details you have regarding that information (this reduces the time spent looking for the information); and
- provide your name, phone number and address for a return reply.
- Your lawyer may also lodge the FOI request or application for you.
