I would like to change information held about me. What can I do?

I receive a government pension. Recently I separated from my husband and I want to go back to my maiden name. How do I change the government records?

  • To change information held about you, you may:
    • telephone the relevant government department or agency and ask them to change your information,
    • see if they provide a form to fill in on their website, or
    • send your request in writing by post or email.
  • You should provide details of:
    • the information that you want changed,
    • your reasons for changing it, and
    • any evidence of why it should be changed.
  • The department or agency will give you an acknowledgement of receipt of your request within 14 days.
  • They will give you their decision on your application which may take up to 30 days.
  • There are no fees for a request to change your information.
